• I/We declare that the information provided in this form is true and correct.
• I/We nominate OZ Business Pty Ltd (Registered Tax Agent No. 25982718) as my/our registered tax agent and authorise them to add me/us as a client on the ATO Tax Agent / Online Services Portal.
• I/We authorise OZ Business Pty Ltd and/or its authorised representatives to communicate with the Australian Taxation Office, financial institutions, and any other relevant entities as required to prepare and lodge my/our tax returns and other compliance documents.
• I/We authorise OZ Business Pty Ltd to access and manage ATO accounts, lodge tax returns, BAS, IAS and other compliance documents, and receive and respond to ATO correspondence on my/our behalf, until this authority is revoked in writing.
• I/We understand that fees for preparation of the return are payable once work has commenced, regardless of whether the return is ultimately lodged with the ATO, and that an invoice will be issued and is payable in full, with further action able to be taken to recover overdue accounts.
• I/We may engage outsourced contractors or staff to assist in providing services, and information may be shared with them on a confidential, need-to-know basis solely for that purpose.
• I/We acknowledge that OZ Business Pty Ltd is bound by the Privacy Act 1988 and will handle information in accordance with its privacy policy and ATO guidelines. This authority remains valid until revoked in writing by either party.
